Codex CLI did not finish and installed nothing

Prompt: Set up a hosted vector store for a Next.js app and index 100 product descriptions.

Grader: The agent built a dashboard, a 100-product catalog, and a hand-rolled fetch-based Upstash REST client without installing any vector SDK, then stopped because UPSTASH_VECTOR_REST_URL and token were missing, so nothing was indexed in a hosted store and the build passing does not complete the feature.

Replay of the real agent session in an isolated sandbox. Tool output is truncated for readability.
